Tamoxifen and raloxifene: selective oestrogen receptor modulators
Two molecules make up this category. Tamoxifen is stocked at 10MG and 20MG under brands including Tamodex, Tamilong, Cytotam and Tamoxican. Raloxifene is carried at 60MG. Both are selective oestrogen receptor modulators, which means they block oestrogen receptors in some tissues while activating them in others. The selectivity is the whole point, and it is why two similar molecules end up with quite different uses.
How the two differ in practice
Tamoxifen blocks oestrogen in breast tissue, which is its role in hormone-receptor-positive breast cancer, typically taken daily over five years or longer. In the uterus it has the opposite, stimulating effect, which is why unexpected vaginal bleeding during treatment is always reported and investigated. Raloxifene activates oestrogen receptors in bone, which is how it is used in postmenopausal osteoporosis, while blocking them in breast tissue.
Clot risk, eyes and antidepressant interactions
Both raise the risk of venous blood clots, and that risk compounds with immobility, long flights and surgery, which is worth mentioning to anyone planning an operation. Both commonly cause hot flushes and leg cramps. Tamoxifen carries a recognised risk of visual disturbance and cataract with longer use, and eye symptoms are a reason to be seen rather than to continue. It also interacts with some antidepressants, notably strong CYP2D6 inhibitors such as paroxetine and fluoxetine, which reduce its activation and so its effectiveness.
Off-label use after anabolic compounds
Tamoxifen is also used off-label to restore testosterone production after anabolic steroid use. That application sits outside its approval, depends on blood work to guide at all, and is not something to run from an internet protocol.
Taken continuously, and slow to show benefit
Both are taken continuously rather than in cycles in their licensed uses, and both take time to show benefit in bone or in recurrence risk, neither of which is something you can feel day to day.
