Levothyroxine, liothyronine and antithyroid medicines

Thyroid treatment splits in two. For an underactive thyroid, levothyroxine is the standard, stocked here from 12.5MCG through 25MCG, 50MCG, 75MCG, 88MCG, 100MCG, 125MCG, 150MCG and 200MCG under brands including Thyronorm, Eltroxin, Thyrox, Lethyrox and Healtroxin. Liothyronine, the more active T3 form, is carried at 5MCG and 25MCG and is much shorter-acting. For an overactive thyroid, methimazole is stocked at 5MG and 10MG.

Micrograms against milligrams

Notice the units. Levothyroxine is dosed in micrograms and methimazole in milligrams, a thousand-fold difference, and the two sit next to each other on the same shelf. Reading MCG against MG on the pack is the single most important check in this category. Levothyroxine strengths are also closely spaced, and 100MCG against 125MCG is a real difference rather than a rounding.

Absorption is unusually sensitive to timing

Absorption is unusually sensitive to what else is in the stomach. Levothyroxine is normally taken on an empty stomach, with water, and separated by several hours from calcium, iron, antacids and some other supplements, all of which bind it and reduce how much gets in. Coffee immediately afterwards reduces absorption as well. Consistency matters more than the particular routine you choose.

Testing, and staying on one brand

Dose is adjusted on blood tests rather than on how you feel, and levels take around six weeks to settle after a change, which is why testing is spaced accordingly. Once stable on a brand and strength, staying with it avoids unnecessary re-testing, since small formulation differences between manufacturers can shift results.

A warning worth memorising

Methimazole carries one warning worth memorising: a sore throat with fever can signal agranulocytosis, a rare drop in white cells, and warrants an urgent blood count rather than waiting it out. Liothyronine's short half-life means it is usually split across the day rather than taken once.
