Corticosteroids stocked at PCTMart
The corticosteroids in this category differ mainly in potency and duration. Prednisolone, stocked at 5MG, 10MG, 20MG and 40MG, is the everyday oral choice for inflammatory flares. Methylprednisolone is somewhat more potent milligram for milligram. Dexamethasone and betamethasone are more potent again and longer-acting. Hydrocortisone sits at the other end, closest to the body's own cortisol, which is why it is used for replacement rather than for suppressing inflammation. Fludrocortisone is carried for its mineralocorticoid effect, and triamcinolone appears in injectable and topical forms.
Potency is not comparable between molecules
Equivalence between these is not intuitive, and the numbers are not comparable: a small number of milligrams of dexamethasone corresponds to a much larger number of prednisolone. Switching between them is a calculation, not a substitution.
Why longer courses are tapered
The most important practical point concerns stopping. Taking a corticosteroid for more than a couple of weeks suppresses the body's own cortisol production, and stopping abruptly leaves you unable to mount a stress response, which can be dangerous during illness, injury or surgery. Longer courses are therefore tapered. If you are on one and become unwell, that is a reason to contact a doctor, not to skip doses.
Dose-related effects, and timing the dose
Dose-related effects include raised blood sugar, raised blood pressure, fluid retention, mood changes and disturbed sleep, with bone density loss, cataract and skin thinning over longer periods. Taking the dose in the morning generally interferes with sleep less. Stomach protection is often added for longer courses.
Topical corticosteroids
Topical corticosteroids follow their own logic. Potency is graded, thinner skin such as the face absorbs far more than palms or soles, and the fingertip unit is the usual way of judging how much to apply. Long runs on the face or in skin folds are avoided because of thinning.
