Andractim is a 2.5% dihydrotestosterone (DHT) gel made in France by Besins Healthcare, supplied in an 80 ml tube. DHT is the strongest natural androgen: it is what testosterone becomes in skin, hair follicles, the prostate and the genitals. Andractim delivers it through the skin, and it ships free to every country we deliver to.

Andractim is prescribed for androgen deficiency in men. Because DHT cannot be converted into oestrogen, doctors also use it for gynaecomastia, where a testosterone product would feed the breast tissue it is meant to shrink. Applied to the chest, it reduces the glandular swelling in many men, most reliably when the gynaecomastia is recent.

It is also used, under medical supervision, where more androgen is wanted at one place on the body. Applied to the face and jaw it thickens beard growth in men whose own DHT is low. Applied to the clitoris it causes clitoral growth, which is why transgender men and some women on testosterone therapy use it. In every one of these uses the gel is applied to a small area in a small amount.

Spread a thin layer on clean, dry skin and let it dry before dressing. The area, the amount and the length of treatment depend on what it is prescribed for, so follow your doctor's instructions and the leaflet in the pack. Wash your hands afterwards. DHT transfers to anyone who touches the treated skin, so cover the area once the gel has dried and keep it away from women who are pregnant or could become pregnant, and from children.

The gel is alcohol based. On the genitals or any broken or sensitive skin it stings for a minute or two after application, and the stinging is worse the more you use. A pea-sized amount is enough for the clitoris; more does not work faster, it only irritates. Do not apply it to mucous membranes, and stop and let the skin recover if it becomes red, dry or sore.

Although the gel is applied to one area, DHT crosses the skin and enters the bloodstream, so the effects are the same as any androgen taken systemically, not only where it was rubbed in. With regular use expect some or all of the following:

• Virilization in women: deeper voice, growth of facial and body hair, clitoral enlargement, acne and oilier skin, and changes to the menstrual cycle. The voice change and the clitoral growth do not reverse after stopping.
• Male-pattern hair loss in anyone with the genetic tendency to it. DHT is the hormone that shrinks scalp follicles, so thinning can start or speed up.
• Acne and oily skin, on the treated area and elsewhere.
• Prostate growth and urinary symptoms in men, and a rise in PSA. Men with a history of prostate problems need a doctor's assessment first.
• Suppression of the body's own testosterone and sperm production while it is used, because the brain reads DHT as a signal that there is enough androgen.
• Lower HDL cholesterol, water retention, raised red blood cell count and mood or libido changes, as with other androgens.
• Local irritation: redness, dryness or itching where the gel is applied.

Androgens are not used by men with prostate or breast cancer, and Andractim must not be used in pregnancy. Your doctor may check your blood count, lipids and prostate during longer treatment.
