En Clofert 50MG (Enclomiphene)

En Clofert 50MG is Maneesh Pharmaceuticals's enclomiphene tablet, 50 mg, in a strip of 5 tablets. Enclomiphene is the active isomer of clomifene, isolated so that the long-lasting, oestrogen-like zuclomiphene isomer is left out.

Enclomiphene is prescribed to men with secondary hypogonadism to raise their own testosterone while keeping sperm production, and is used in some fertility clinics for the same purpose. It is not approved by the FDA or EMA as a standalone drug, and is available as an Indian generic.

Like clomifene, enclomiphene blocks oestrogen receptors in the hypothalamus so the pituitary releases more LH and FSH, which raise testosterone and support sperm production. Because it contains none of the zuclomiphene isomer it clears from the body within days and causes fewer oestrogen-like effects, such as mood changes and visual symptoms, than clomifene.

It is taken as a tablet once a day, or on alternate days, at a dose set from testosterone and oestradiol blood tests, for as long as the doctor prescribes. Follow the prescription; raising the dose beyond the point where testosterone is in range only raises oestradiol.

• Headache and hot flushes.n- Nausea and diarrhoea.n- Acne.n- Irritability, mood changes or trouble sleeping.n- A rise in oestradiol with breast tenderness.n- Visual symptoms, less often than with clomifene but a reason to stop.

Enclomiphene is for men; it is not used in women or in pregnancy. It is not appropriate in primary hypogonadism, where the testes cannot respond to LH, which is why a blood workup comes first. Testosterone, oestradiol, haematocrit and, where fertility matters, semen analysis are monitored.
